Submit Your Form
Attendance forms are due by the 15th of the month following care, no later than 5:00 p.m. If the 15th falls on a weekend or holiday, forms are due the last working day before.

Accuracy Matters
Forms must be filled out completely, with exact in/out times recorded daily. Parent and provider signatures are required on the billing side after the last day of care.

Processing Your Payment
Child Action issues payments within 21 days of receiving a complete attendance form. Forms are processed in order received. Submissions after business hours are marked received the next business day.

Late Forms
Forms received after the 25th of the following month may not be paid. Incomplete, pre-signed, or altered forms (including the use of white-out) will not be accepted.
