Submit or Update Your Rates
Once you’ve established your rates, submit the appropriate Provider File Certification Form along with your current rate sheet. Forms are available in multiple languages.
Email both documents to info@childaction.org. Your rate sheet must include your name, address, license number, date, and signature.
